Our residential program covers key areas such as community access, money management goals, mental health optimization, expression of feelings and needs, informed decision-making, representative payee support, and the development of close relationships with care providers.
Host Homes
We offer secure and nurturing host home placements with dedicated providers for adults on the waiver. Our tailored approach caters to individuals with diverse intellectual, developmental, and mental health needs, ensuring a personalized fit between our individuals and host homes.
PCA Setting
Our residential services extend to individuals having their own room in a house setting, with 24/7 rotating staff members available for personal care assistance, support, and supervision.
Family Caregiver Model
Individuals receiving care through our family caregiver support model remain in the comfort of their own homes, with family members providing services. Designated family caregivers receive training and support from SY Services, following the same care standards as our host home providers.
Respite
We offer respite care to provide short-term relief for families and providers, acknowledging the challenges and emotional strain caregivers often face. This service also offers individuals receiving care a break from their routine environment, allowing them to recharge.
